Job summary

U.S. Department of Veterans Affairs in Manchester is seeking a Registered Nurse - Primary Care Care Manager. You will provide fundamental primary care coordination, support transitions of care, and deliver preventive and chronic disease management using VA guidelines.

Engagement in interdisciplinary teamwork, patient education, and evidence-based practices will be required, with a schedule Monday–Friday in outpatient settings at the Manchester VA.

Qualifications

• English language proficiency required.
• State-licensed RN in the United States.
• Graduate of an approved nursing program (ACEN/CCNE).
• Bridge programs allowing nursing licensure prior to completion acknowledged.

Responsibilities

• Provide fundamental primary care coordination for PACT.
• Support transitions of care and patient-centered nursing care.
• Deliver preventive and chronic disease management using VA/DOD guidelines.
• Coordinate referrals, labs, and follow-ups within the care team.
• Engage in huddles and panel management to improve outcomes.
