Job summary

Responsibilities

• Organize and direct team members to achieve patient outcomes.
• Coordinate care within the Primary Care Clinic using the PACT model.
• Utilize nursing process and evidence-based practice to develop patient care plans.
• Triage and resolve patient care problems with the interdisciplinary team.
• Coordinate resources and referrals to support discharge and health maintenance.
