Job summary

Department of Veterans Affairs seeks a Registered Nurse Care Manager to deliver fundamental, knowledge-based nursing care across panels of patients. The role emphasizes primary care coordination, transitions of care, and proactive chronic disease management for veterans via in-person, telephonic, and virtual channels.

Preferred candidates have 2+ years of RN experience including primary care exposure, with an active nursing license and compliance with VA practice guidelines.

Qualifications

• English language proficiency required for written and spoken communication.
• Graduate from an approved nursing program with active state licensure.
• Bridge program options for master's level entry nursing candidates.
• Foreign nursing graduates require degree equivalency validation.
• Current, full, active, unrestricted registration in a US state or DC.
• Preferred: 2 years RN experience with 1 year in primary care/outpatient.

Responsibilities

• Provide fundamental primary care coordination for the PACT.
• Support transitions of care and veteran-centered nursing care.
• Deliver preventive and chronic disease management interventions.
• Use VA/DOD guidelines and health coaching for patient self-management.
• Coordinate care across acute and ambulatory settings.
