Job summary

SFP Health Group is seeking a Referral Coordinator – Primary Care to manage and track patient referrals, coordinating between providers, patients, and payers. The role emphasizes HIPAA-compliant handling and timely, accurate processing to keep patients within the network.

The ideal candidate will have 2+ years in referral coordination, proficiency with Medicare/Medicaid lines, and experience with Excel and Availity, plus familiarity with Athena EMR systems.

Qualifications

• Minimum 2 years of experience as a Referral Coordinator or similar role.
• Experience with Medicare, Medicaid, Managed Care, Commercial, and Workers’ Compensation.
• Proficient in Excel and Availity.
• Knowledge of CPT, ICD-10, and medical terminology.
• Bilingual skills are a plus.

Responsibilities

• Receive, review, and track referral requests to specialists and diagnostic services.
• Schedule patient appointments with specialists and confirm referrals.
• Follow up with patients and providers to ensure timely delivery of care.
• Maintain updated records of referrals in the EMR and tracking systems.
• Create and maintain the SFP Health Group Preferred Provider Network list.
