Millennium Physician Group in Jacksonville, FL is seeking a General/Primary Care physician to deliver comprehensive, evidence-based care across the lifespan in our ambulatory primary care practice. You will evaluate, diagnose, and manage acute and chronic conditions, provide preventive services, and coordinate care with interdisciplinary teams to improve patient outcomes within a value-based care model.

Responsibilities

• Evaluate, diagnose, and manage a broad range of primary care conditions including hypertension, diabetes, hyperlipidemia, COPD, thyroid disorders, obesity, depression, anxiety, infections and more, following evidence-based guidelines.
• Order, interpret, and integrate laboratory studies, imaging, ECGs, and point-of-care tests to guide treatment decisions.
• Develop and oversee individualized treatment plans including medication management, preventive services, immunizations, and chronic disease management.
• Coordinate care with specialists, APPs, nurses, pharmacists, care managers, social workers, and other professionals to ensure comprehensive patient care.
• Conduct annual wellness visits and preventive services, promote health maintenance, and support population health initiatives.
• Document encounters and outcomes in the EHR and participate in quality improvement, clinical pathways, and continuing medical education to enhance primary care delivery.
