Responsibilities

• Evaluate, diagnose, and manage patients with acute illnesses, chronic diseases, and preventive care.
• Develop and oversee individualized treatment plans including medications, screenings, immunizations.
• Coordinate care with APPs, nurses, pharmacists, and specialists to improve outcomes.
• Conduct annual wellness visits and preventive screenings in a primary care setting.
• Document encounters and outcomes in the electronic health record per regulatory standards.
