Job summary

HealthTrac at Health Solutions in Pueblo, Colorado is seeking a full-time Primary Care Physician to join our Medical Services Team. You will provide comprehensive primary care for pediatric and adult patients, coordinate with a multidisciplinary team, and deliver preventive services aligned with PCMH principles.

Responsibilities include health assessments, ordering and interpreting tests, and creating individualized treatment plans.

Qualifications

• Applicants must be licensed to practice medicine in the State of Colorado as an MD or DO.
• Board Certification in Family Medicine or Internal Medicine is preferred.
• D.E.A. registration in good standing.
• Current CPR or ACLS within 3–12 months of employment, if appropriate.
• Must be insurable under company-provided malpractice coverage.
• Computer proficiency.

Responsibilities

• Provide comprehensive primary care across the lifespan, including health assessments and management of acute and chronic conditions.
• Order, perform, and interpret diagnostic tests to support diagnoses.
• Develop and adjust treatment plans, prescribe medications, and deliver preventive interventions.
• Coordinate referrals and transitions of care within a multidisciplinary team.
• Maintain accurate electronic health records and ensure quality documentation.

Education

Licensed medical doctor (MD) or Doctor of Osteopathic Medicine (DO) in Colorado

Board Certification in Family Medicine or Internal Medicine preferred
