Job summary

U.S. Department of Veterans Affairs in the United States seeks a physician to provide comprehensive primary care within the Patient Aligned Care Team (PACT) model.

The incumbent will coordinate with DoD and VA providers to ensure seamless transitions and continuity of care for a panel of adult patients. Ideal candidates have MD/DO, must hold an active US medical license, and may be Board Eligible or Board Certified in Internal or Family Medicine.

Qualifications

• MD/DO degree and US license in good standing.
• Board eligibility or board certification preferred.
• Experience with CPRS or VA electronic health records and veteran health issues is a plus.

Responsibilities

• Provide comprehensive Primary Care using the PACT model.
• Prescribe medications and manage patient care.
• Coordinate with DoD and community providers for smooth transitions of care.
• Contribute to achieving performance targets for access, quality, and patient satisfaction.
