Qualifications

Responsibilities

• Provide comprehensive primary care and preventive services within the PACT framework.
• Manage adult patients with acute and chronic conditions
• Collaborate with the health care team and coordinate care across services
• Document care and adhere to VA clinical guidelines in the EHR
• Engage in continuous quality improvement and patient education.
