Qualifications

• 3 years primary care experience preferred.
• Active, non-probationary state Physician Assistant license is required.
• Active DEA license required.
• US work authorization required.
• Employment in outpatient clinics on a collaborative care team.

Responsibilities

• Provide exceptional primary care within a value-based model.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with physicians, NPs, nurses, and care managers.
• Lead population health efforts in collaboration with the care team.
