Job summary

Oak Street Health in Tucson, AZ seeks a Physician Assistant to deliver effective and equitable value-based primary care to adults on Medicare. You will work within a collaborative care team and provide longitudinal care, building trust and ensuring patients understand their care plans.

You will practice in outpatient clinics with a physician-led team, focusing on primary care, care coordination, and population health to improve outcomes.

Qualifications

• 3 years primary care experience preferred.
• Active, non-probationary Physician Assistant license.
• Active DEA license.
• US work authorization.
• Someone who embodies being Oaky.

Responsibilities

• Provide exceptional primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with physicians, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Educate patients on health conditions, care plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
