Job summary

Sycamore Physician Contracting LLC is seeking a permanent Primary Care Physician for outpatient care in Alabama, focusing on senior and Medicare Advantage patients. The role emphasizes preventive care, quality outcomes, and relationship-based care with team support.

Schedule features no nights or weekends and no hospital rounding. You will manage chronic conditions such as diabetes, hypertension, COPD, heart failure, and CKD with a small panel approach and tight follow-up to optimize outcomes.

Qualifications

• Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatrics.
• Active Alabama license or ability to obtain.
• Preferred: 3+ years of outpatient primary care experience.
• ACLS/BLS as required for the assignment.

Responsibilities

• Provide outpatient primary care for seniors and Medicare Advantage patients.
• Deliver preventive care and drive quality outcomes.
• Collaborate with care coordination, pharmacy, and administrative support.

Requirements

• Active Alabama license or ability to obtain.
• Preferred: 3+ years of outpatient primary care experience.
• Preferred: Medicare Advantage or value-based care experience; this experience is not required.
• ACLS/BLS as required for the assignment.

Compensation and logistics

• Salary with a quality-based performance bonus; compensation is not RVU/production driven.
