Qualifications

• MD/DO degree required and residency completed in Family or Internal Medicine.
• CA medical license active or able to obtain before start date.
• CA DEA certificate and BLS certification required or obtainable.

• Conduct consultations, follow-ups, well-checks, and screenings.
• Care for adults and seniors with a dedicated support staff.

This role allows you to build meaningful, long-term relationships with patients while practicing value-based, patient-centered care supported by an integrated care model. Required Qualifications:

• Doctor of Medicine (M.D.) or Doctor of Osteopathic (D.O.)


• Completion of an accredited residency in Family Medicine or Internal Medicine


• Board Certification in Family Medicine or Internal Medicine, or Board Eligible for medical residents


• Active and unrestricted California Medical License or ability to obtain prior to start


• Current California DEA certificate or ability to obtain prior to start


• Current Basic Life Support (BLS) certification or ability to obtain prior to start

For California Residents Only: The salary range for California residents is $260,000-$360,000 per year. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement.
