Conviva Primary Care, a Humana subsidiary, seeks a Primary Care Physician for the Pompano East, Florida clinic. The clinician will deliver comprehensive outpatient care with a focus on disease prevention and health maintenance, documenting medical histories and coordinating with specialists as needed.

Onsite in Pompano East, the role reports to the Associate Medical Director and involves a hands-on approach with seniors, offering longer patient visits and engaged care within a value-based model.

Formación

• Doctor of Medicine or Osteopathic Medicine.
• Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine.
• Valid, unrestricted license in the state of desired employment (or willing to obtain).
• Current, unrestricted DEA license.

Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic at our Pompano East, Florida clinic. You will have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are affecting health outcomes. As a Primary Care Physician, you will report to our Associate Medical Director. Your role will be onsite and in-person at the Pompano East clinic.

Job Description

The Physician focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention. Keeps a medical history and medical records. Refers the patient to specialists as needed. Begins to influence department’s strategy. Makes decisions on moderately complex to complex issues regarding technical approach for project components, and work is performed without direction. Exercises considerable latitude in determining objectives and approaches to assignments.

A day in the life of a Conviva physician

• Monday-Friday 8:00am-5:00pm
• Join an experienced team with an established patient panel
• See 12-15 patients per day. Opportunity to spend more time with patients
• Longer authorized appointments (45 minutes)
• Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care

Required Qualifications

• Doctor of Medicine or Osteopathic Medicine
• Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
• Valid, unrestricted license in the state of desired employment (or willing to obtain)
• Current, unrestricted DEA license

Preferred Qualifications

• Minimum of two+ years of experience
• 1+ years of experience managing Medicare Advantage panel of patients
• Knowledge of Medicare guidelines and coverage.
• Knowledge of HEDIS quality indicators.
• Fluent in Haitian Creole and English

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$255,000 - $298,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
