Conviva Senior Primary Care in Palm Coast, FL seeks a Primary Care Physician to join on-site at our clinic. You will report to the Associate Medical Director and see patients Monday through Friday, 8:00 am to 5:00 pm, with longer 45-minute appointments as needed.

We offer a guaranteed base salary with a bi-annual bonus, a robust benefits package, CME allowance, malpractice coverage, relocation/sign-on options, and a 401(k) plan, supporting a comprehensive, patient-centered approach.

Formación

• MD or DO degree from an accredited medical school.
• Board certification or eligibility in geriatrics, family medicine or internal medicine.
• Active Florida license or eligibility to obtain.
• Current, unrestricted DEA license.
• Minimum of two years of clinical experience; 1+ year managing Medicare Advantage panel.

Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in Palm Coast, FL.

As a Primary Care Physician, you will report to the Associate Medical Director. You will be on-site at our Palm Coast, FL. clinic.

A day in the life of a Conviva physician

• Monday-Friday, 8:00 am-5:00 pm
• See an average of 15 patients per day
• Longer authorized appointments (45 minutes)
• You will be on-site

Required Qualifications

• Doctor of Medicine (MD) or Osteopathic Medicine (DO)
• Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
• Valid, unrestricted license in the state of Florida or able to obtain one.
• Current, unrestricted DEA license
• Screened for TB
• Minimum of two years of experience
• 1+ years of experience managing a Medicare Advantage patients panel
• Associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$255,000 - $298,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
