Job summary

Conviva Primary Care, a Humana Inc. subsidiary, seeks a Primary Care Physician for our Plantation, FL clinic. You will focus on outpatient medicine, continuity of care, and disease prevention, while addressing social determinants of health.

The role is onsite with an established patient panel and a 40-hour work week. You will report to the Associate Medical Director and participate in an TB screening program as part of Humana/Senior Bridge.

Qualifications

• MD/DO board certified or board eligible in Geriatric, Family or Internal Medicine.
• Valid, unrestricted medical license in the state of employment.
• Active and unrestricted DEA registration.

Responsibilities

• Provide outpatient primary care focused on prevention and health maintenance.
• Maintain patient medical history and coordinate referrals.
• Deliver care in an on-site clinic and participate in team-based care.

Become a part of our caring community Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in Plantation, Florida. You will have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are affecting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a versatile approach. As a Primary Care Physician, you will report to our Associate Medical Director. This role will be onsite and in-person our Conviva Pine Island in Plantation FL. As the Primary Care Physician you will focus on outpatient medicine, continuity of care, health maintenance, and disease prevention. You will keep a medical history and medical records. Refer patients to specialists. Influence clinic department's strategy. You will make decisions on moderately complex to complex issues. You will have considerable latitude in determining best approaches to assignments.

A day in the life of a Conviva physician Monday-Friday 8:00am-5:00pm Join an experienced team with an established patient panel See 12-15 patients per day. Opportunity to spend more time with patients Longer authorized appointments (45 minutes) Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care.

Use your skills to make an impact This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $255,000 - $298,500 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.

Required Qualifications

• Doctor of Medicine or Osteopathic Medicine Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
• Valid, unrestricted license in the state of desired employment (or willing to obtain)
• Current, unrestricted DEA license

Preferred Qualifications

• Minimum of two+ years of experience
• 1+ years of experience managing Medicare Advantage panel of patients
• Knowledge of Medicare guidelines and coverage.
• Knowledge of HEDIS quality indicators.
• Bilingual Spanish and English a plus
