Job summary

Mercy Clinic Primary Care is seeking a Primary Care Nurse to triage calls, support Providers, and help manage an attributed patient panel. The role emphasizes patient-centered care within Mercy’s values and service standards.

Newly created position with on-site training followed by a 50/50 hybrid model. On-site schedule is Mon–Fri 0800–1630, no nights or weekends, with monthly on-site meetings.

Qualifications

• LPN diploma with active license and 2–3 years' experience preferred.
• RN with ASN/BSN or 4 years' experience in lieu of BSN and active RN license.
• Experience helpful in a clinic setting and patient triage.

Responsibilities

• Manage clinic triage calls/messages and support Providers.
• Collaborate with care team to deliver patient-centered care.
• Assist with transitioning to a hybrid work model after training.
