Qualifications

• Diploma in Practical Nursing from an accredited program.
• Active LPN license to practice.

Responsibilities

• Manage triage calls/messages for the clinic.
• Collaborate with Providers and care team to coordinate patient care.
• Support on-site operations with training prior to hybrid work shift.
• Transition to a 50/50 hybrid work model after on-site training.
• Attend monthly on-site clinic meetings as required.
