Job summary

Fairview Health Services in Maple Grove is seeking a Nurse Practitioner to join the M Health Fairview Clinic – Bass Lake, a growing primary care practice serving patients of all ages. The clinic emphasizes collaborative, patient-centered care with a Fully integrated EPIC system.

This full-time position offers a flexible schedule within a transitioning Bass Lake clinic expansion, and opportunities for professional development in a supportive, team-based environment.

Qualifications

• Graduate of a school of nursing and accredited formal Family Nurse Practitioner training.
• BC/BE through the AANP or ANCC.
• Licensed or ability to obtain APRN license with the Minnesota Board of Nursing.
• Ability to obtain and/or maintain DEA certification for the State of Minnesota.
• Meet threshold criteria/qualifications for Credentialing and Privileges (if required).
• Preferred: 2+ years of Nurse Practitioner experience.

Responsibilities

• Assess health status for specific patient populations using advanced practice knowledge.
• Obtains relevant health and medical history and performs physical exams.
• Develops and implements an individualized plan of care with collaboration.
• Prescribes pharmacologic and non-pharmacologic therapies.
• Communicates with patients and families about diagnosis, treatment, and follow-up.
• Coordinates with care team and participates in quality and regulatory activities.
