Job summary

Fairview Health Services invites applications for a Nurse Practitioner in its M Health Fairview Clinic – Wyoming, a primary care setting. This 1.0 FTE role features a collaborative team, open scheduling (0.8–1.0 FTE), and seamless EPIC electronic medical records integration.

Daily responsibilities include patient assessments, history taking, physical exams, ordering tests, and developing care plans. You will collaborate with physicians, prescribe medications, and provide education to patients

Qualifications

  • Graduate of an accredited nursing program with formal nurse practitioner training.
  • BC/BE through the AANP or ANCC is required.
  • Licensed or eligible for Minnesota APRN license.
  • DEA certification for Minnesota required/able to obtain.

Responsibilities

  • Assess health status for specified patient populations using advanced practice skills.
  • Obtain medical history and conduct focused physical examinations.
  • Order and interpret diagnostic tests and develop an individualized care plan.
  • Collaborate with physician and care team; ensure effective care coordination.
  • Prescribe medications and advise on treatment options.
  • Provide patient and family education and follow-up planning.

Fairview is seeking a Nurse Practitionerto join our primary care practice atM Health Fairview Clinic – Wyoming.Join a collaborative and growing practice with a supportive, experienced care team dedicated to providing high-quality, patient-centered care.

Practice Details
  • Established primary care practice with a collaborative team-based environment
  • Patient encounters average 18 patients per day
  • 1.0 FTE = 34 patient contact hours per week
  • Openings: 0.8 to 1.0 FTE (Full Time) – flexible scheduling options
  • Fully integrated Electronic Medical Record (EPIC)
Responsibilities
  • Assess health status for specific patient populations using advanced practice knowledge and skills.
  • Obtains relevant health and medical history.
  • Performs a physical examination based on age and medical history.
  • Performs/orders preventative and diagnostic procedures based on the patient's age and history.
  • Identifies health and medical risk factors.
  • Identifies acute and chronic health problems.
  • Appropriately orders and accurately interprets diagnostic results.
  • Develops and implements an individualized plan of care.
  • Establishes practice agreements with collaborating physician.
  • Collaborates with health care team members in developing realistic care expectations.
  • Determines effectiveness of treatment plan with documentation of patient care outcomes.
  • Revises and modifies the care plan as needed to achieve health and medical goals.
  • Refers care to other health care professionals and community agencies as needed.
  • Prescribes pharmacologic (including controlled substances) and non-pharmacologic agents.
  • Communicates effectively with patients and family for understanding of diagnosis, treatment plan, and need for follow-up care.
  • Provides education to patients, families or caregivers as needed.
  • Documents according to established guidelines to facilitate ongoing quality care in a timely manner.
  • Responds in a timely manner to phone calls regarding patient needs.
  • Contributes to effective and efficient practice management policies and practices that support patient access to medical care.
  • Participates in access planning for applicable area or population.
  • Supports appropriate billing for professional service through timely documentation and maintaining adequate knowledge of FVC billing policies and procedures.
  • Adheres to all relevant policies, procedures, and practice guidelines.
  • Assists with the preparation for JCAHO and other regulatory surveys.
  • Manages patient care through appropriate use of available resources.
  • Appropriately prescribes and orders ancillary services.
  • Utilizes Fairview referral network appropriately.
  • Manages chronic health/disease conditions.
  • Provides leadership for clinical and support staff.
  • Serves as a resource to support staff, enhancing quality of care.
  • Participates in the training, orientation, and evaluation of clinical and support staff.
  • Participates in appropriate clinical investigation relevant to specific patient population.
  • Responds to nursing staff educational needs in formal and informal venue demonstrating nursing procedures, charting procedures, and reviewing pathophysiology of disease and new treatment modalities.
  • Serves as consultant and role model for nursing staff.
  • Maintains competence and professional development. Engages in continuing education.
  • Is active in professional and other appropriate organizations.
  • Demonstrates behaviors that support performance improvement activities.
  • Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.

Required Qualifications
  • Graduate of an accredited school of nursing and formal nurse practitioner training program

  • Licensed or ability to obtain APRN license with the Minnesota Board of Nursing
  • Ability to obtain and /or maintain DEA certification for the State of Minnesota
  • Licensed or ability to obtain Minnesota RN licensure and Advanced Practice Nurse Prescriber (APNP) certification through the Minnesota Board of Nursing
  • Ability to obtain and/or maintain DEA certification for the State of Minnesota
  • Meet threshold criteria/qualifications for Credentialing and Privileges

Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Positions with the same min/max are flat rate jobs based on 40 hours/week. Compensation plans based on productioncan allow for higher pay than the range posted.
