Job summary

Fairview Health Services is seeking a Physician Assistant for a primary care practice at M Health Fairview Clinic – Bass Lake in Maple Grove, MN. The clinic will relocate to a larger site in 2027, with a collaborative, multidisciplinary team and EPIC EMR.

The role supports 18–20 patient encounters per day, 1.0 FTE with flexible scheduling, and a range of medical services in a growing northwest metro setting.

Qualifications

• Graduate of an accredited PA training program.
• Current licensure with the Minnesota Board of Medical Practice.
• Certification by the NCCPA.
• Approved or eligible for prescriptive authority.
• Meets criteria for credentialing and privileging.

Responsibilities

• Assess patients’ health status, diagnose, and implement care plans.
• Provide medical care within scope of practice per supervising physician.
• Discuss treatment options appropriate for age and culture.
• Manage care and monitor progress after initiating treatment.
• Communicate clearly with patients and families about diagnoses and follow-up.
• Educate patients to participate in their own care and provide education materials.
• Document per guidelines to support quality of care.
• Delegate tasks to appropriate staff and respond to emergencies.
