Fairview Health Services is seeking a Nurse Practitioner to join our primary care team at M Health Fairview Clinic – Midway as a Same Day Provider. You will deliver timely, patient-centered care for acute illnesses and urgent health concerns in an outpatient setting, collaborating with physicians and care teams.

This full-time role features 34 patient contact hours per week, with about 18–22 encounters per day, within a 12-provider primary care practice.

Qualifications

• Graduate of an accredited school of nursing and formal nurse practitioner training program.
• Board-certified or eligible as a Nurse Practitioner (AANP/ANCC).
• Licensed or eligible to obtain Minnesota APRN license.
• DEA certification for the State of Minnesota.
• Meet credentialing and privileging requirements.

Responsibilities

• Provide timely, patient-centered care for acute illnesses, minor injuries, and urgent health concerns in outpatient settings.
• Collaborate with physicians and care teams to ensure prompt access and continuity of care.
• Manage a full-time schedule with 34 patient contact hours per week and ~18–22 patient encounters per day.
• Utilize EPIC EMR and Nabla AI documentation support to streamline charting and care coordination.

Fairview is seeking a Nurse Practitionerto join our primary care team at M Health Fairview Clinic – Midway as a Same Day Provider. As a Same Day Nurse Practitioner, you will provide timely, patient-centered care for acute illnesses, minor injuries, and urgent health concerns in an outpatient setting. Working collaboratively with physicians and care teams, you will help ensure patients receive prompt access to care while supporting continuity and coordination of treatment. This is an excellent opportunity to join a thriving practice with multiple on‑site specialties and a long‑tenured, highly experienced support staff.

Practice Details

• 1.0FTE (Full Time) - Flexible schedule
  • 34 patient contact hours per week
• 12 provider primary carepractice with a mix of both Family Medicine and Internal Medicine providers
• Patient encounters average 18-22 patients per day
• As part of Fairview’s Accountable Care Organization (ACO), providers practice in a collaborative, team-based model focused on quality outcomes, care coordination, and population health management.
