Job summary

Fairview Health Services in Maple Grove is seeking a Family Medicine physician to join our primary care practice at M Health Fairview Clinic – Bass Lake. The clinic provides care for patients of all ages with a patient-centered, team-based approach, and uses EPIC for charting.

Open 1.0 FTE or less, with flexible scheduling; anticipated to move to a larger Bass Lake location in January 2027 and benefits include medical, dental, vision, PTO, CME, and retirement plans.

Qualifications

• Graduate of an accredited ACGME/AOA Family Medicine residency program.
• BC/BE with ABMS or AOA.
• Licensed or ability to obtain licensure through Minnesota Board of Medical Practice.

Responsibilities

• Provide family medicine care to patients of all ages.
• Collaborate with multidisciplinary care team.
• Utilize EPIC for charting and documentation.
