Job summary

Conviva Primary Care in Pompano Beach, FL is seeking an Advanced Practice Provider (APP) to deliver comprehensive primary care for adults and seniors. You'll work in a team-based, value-driven model with longer appointments and emphasis on social determinants of health.

Expect 12-15 patients/day, 45-minute visits, and direct clinical focus. Qualifications include an unrestricted NP license, national certification (AANP/ANCC), and Medicare Advantage experience.

Qualifications

• Active, unrestricted Nurse Practitioner license in the state of practice.
• National certification (AANP or ANCC).
• DEA license (or ability to obtain).
• Minimum of two+ years of experience.
• 1+ years managing Medicare Advantage panel of patients.
• Knowledge of Medicare guidelines and coverage.
• Knowledge of HEDIS quality indicators.
• Fluent in Creole and English.

Responsibilities

• Provide comprehensive primary care services to adult and geriatric patients, including assessments and ongoing care management.
• Manage chronic conditions with emphasis on preventive care and wellness.
• Perform exams, order/interpret tests, and develop treatment plans.

• Educate patients and caregivers on disease management and healthy lifestyle choices.
• Maintain accurate EMR documentation.

Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care Advanced Practice Provider (APP) for our clinic in Pompano East, Florida. You will have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are affecting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a versatile approach. As a Primary Care Physician, you will report to our Associate Medical Director.

Key Responsibilities

• Provide comprehensive primary care services to adult and geriatric patients, including assessments, diagnosis, treatment, and ongoing care management
• Manage chronic conditions with an emphasis on preventive care and wellness
• Perform physical exams, order and interpret diagnostic tests, and develop individualized treatment plans
• Collaborate closely with physicians, care coordinators, behavioral health specialists, and clinical support staff
• Focus on quality-driven outcomes, including HEDIS measures, CMS Star initiatives, and population health goals
• Educate patients and caregivers on disease management, medications, and healthy lifestyle choices
• Maintain accurate and timely documentation in the electronic medical record (EMR)
• Participate in care team meetings and quality improvement initiatives

A day in the life of a Conviva APP

• Monday-Friday 8:00am-5:00pm
• Join an experienced team with an established patient panel

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
