Job summary

Conviva Senior Primary Care seeks a Nurse Practitioner or Physician Assistant to join our Edgewater clinic in a full-time role. You will work with a multidisciplinary team to deliver preventive and chronic care tailored to seniors.

The position emphasizes value-based care, collaboration, and on-site presence, with a strong focus on quality outcomes and patient-centered planning. A Florida license and board-certified nursing credentials are required.

Qualifications

• Master’s degree in nursing with board certification required.
• Active, unrestricted NP license in Florida or PA license as appropriate.
• Experience with value-based care and Medicare Advantage settings preferred.
• Active DEA license and ability to obtain Medicare/Medicaid numbers if needed.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support growth plans at the center.
• Work on-site and contribute to value-based care initiatives.

Humana’s Primary Care Organization is seeking a full-time Primary Care Nurse Practitioner (NP), or Physician Assistant (PA) to join our team at our clinic located in Edgewater, FL

As a Nurse Practitioner (NP) or a Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, diverse team. This team includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

What We’re Looking For

Required:

• Master’s degree in nursing with board certification.
• Active, unrestricted NP, or PA license in the state of Florida.
• Experience in value-based care, primary care or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
