Oak Street Health is seeking a Nurse Practitioner to deliver effective and equitable value-based primary care to adults on Medicare in outpatient settings. You will work within a collaborative care team including physicians, PAs, and support staff to provide longitudinal care and build strong patient relationships, enabling better outcomes and patient engagement.

You will see patients independently, coordinate with specialists and hospitals as needed, and participate in care management

Formación

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in at least one NP specialty (Family/Adult-Gerontology/Gerontological).
• Active, non-probationary NP license in the US; Active DEA license.

Responsabilidades

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with other providers, specialists, facilities, and agencies.
• Lead population health efforts with the Care Team.
• Assist with phone triage and outreach.
• Educate patients on health conditions, care plans, and treatments.
• Participate in promotional activities.
• Conduct home visits as needed.
• Other duties as assigned.
