Job summary
Oak Street Health in Harrisburg seeks a Nurse Practitioner to deliver value-based primary care to adults on Medicare in a collaborative outpatient setting. This full-time role requires an MSN/NP credential, state NP license and DEA, with a focus on comprehensive wellness visits, care coordination, and population health across our care team.
You will conduct home visits as needed and participate in promotional activities to support community health efforts.
Qualifications
- MSN or NP doctorate/certificate program graduate.
- Active state NP license in PA.
- Active DEA license.
- US work authorization.
Responsibilities
- Provide exceptional primary care.
- Conduct office visits for routine and acute issues.
- Administer Annual wellness visits and HRAs.
- Care coordination with providers, specialists, facilities and agencies.
- Population health leadership with the care team.
- Assist with phone triage and outreach.
- Educate patients on health conditions and care plans.
- Participate in Oak Street Health promotional activities.
- Conduct home visits as needed.
- Other duties as assigned.
