Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US Work Authorization
• National certification in one of the following specialties: Family NP, Adult-Gerontology Primary Care NP, Adult NP, Gerontological NP

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with physicians, specialists, testing facilities, and agencies.
• Lead population health initiatives with the care team to improve outcomes.
• Assist with phone triage and outreach to patients.
• Educate patients on health conditions and treatment plans.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Perform other duties as assigned.
