Job summary

Bookmark Medical in Buckeye, AZ is seeking a Nurse Practitioner or Physician Assistant to join our established primary care team. You will provide comprehensive primary care services, perform patient evaluations, and develop individualized treatment plans in a collaborative setting.

The role emphasizes preventive care, chronic disease management, and using EHR systems to document care. A state license, NP/PA program completion, and at least one year of primary care experience are required.

Qualifications

• Completion of an accredited NP or PA program.
• Current, unrestricted NP or PA license in the practicing state.
• Minimum one year of primary care clinical experience.
• Strong clinical judgment and patient-centered care.
• Ability to work collaboratively in a team-based setting.
• Experience using electronic health record systems.

Responsibilities

• Provide comprehensive primary care services including preventive and chronic disease management.
• Perform patient evaluations, exams and diagnostic assessments.
• Develop and implement individualized treatment plans.
• Order, review, and interpret diagnostic tests and lab results.
• Educate patients on preventive health and treatment options.
• Collaborate with care team to coordinate patient care.
• Maintain accurate documentation in EHR.
