Job summary

Bookmark Medical is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our established primary care team in Buckeye, AZ.

Provide comprehensive primary care services, including preventive care, acute visits, and chronic disease management. You will evaluate patients, develop and implement treatment plans, order and review tests, and coordinate with physicians and specialists while maintaining complete EHR documentation in a collaborative, value‑driven environment.

Qualifications

• Completion of NP or PA program from an accredited institution.
• Current, unrestricted NP or PA license in the state of practice or eligibility to obtain licensure.
• Minimum of one year of clinical experience in primary care.
• Strong clinical judgment and patient-centered approach.
• Ability to collaborate with providers and use EHR systems.

Responsibilities

• Provide comprehensive primary care services including preventive, acute, and chronic care.
• Perform patient evaluations, physical exams, and diagnostic assessments.
• Develop and implement individualized treatment plans based on findings.
• Order, review, and interpret diagnostic tests and labs.
• Educate patients on prevention and disease management and coordinate care with the team.
• Maintain timely, accurate documentation in the electronic health record.
