Job summary
Bookmark Medical in Raynham, MA is seeking a Primary Care Advanced Practitioner (NP/PA) to join our established Primary Care team. You will provide comprehensive care to a diverse patient population and develop personalized treatment plans in collaboration with specialists.
The role offers a base salary of $120,000–$145,000 with incentives, a transparent value-based compensation model, and a comprehensive benefits package including medical, dental, vision, 401(k) with company match, CME
Qualifications
- Graduate of an accredited NP or PA program with current certification and licensure.
- At least one year of APP experience, preferably in primary care.
- Strong clinical skills, collaborative mindset and EMR proficiency.
Responsibilities
- Provide comprehensive care to a diverse patient population.
- Conduct thorough physical exams, diagnose conditions and tailor treatment plans.
- Promote health education and disease prevention.
- Order, interpret diagnostics and manage chronic conditions.
- Collaborate with specialists to ensure coordinated care.
- Maintain accurate medical records and patient charts.
- Stay current with medical advancements in Primary Care.
