Job summary

Millennium Physician Group in Florida is seeking a Primary Care Advanced Practice Provider (APRN) to deliver comprehensive, patient-centered care across the continuum, collaborating with physicians and an interdisciplinary team.

In this role, you will manage acute, chronic, and palliative needs, conduct assessments, develop care plans, interpret results, prescribe medications, and coordinate services to improve outcomes in a value-based care model.

Qualifications

• Master’s or Doctor of Nursing Practice degree from an accredited program.
• Current, unrestricted NP license and APRN licensure in the applicable state.
• National board certification (FNP/AGPCNP/AGACNP) or equivalent.
• DEA registration and BLS certification required.
• ACLS certification preferred.
• Two+ years of APN primary care experience preferred.

Responsibilities

• Total care of all assigned patients in collaboration with the community-based physician and care team.
• Assessments, care planning, risk management, and provision of acute, chronic, and palliative care within scope.
• Document all active medical conditions timely in the EMR per MPG policy and CMS guidelines.
• Lead in applying the nursing process to improve outcomes and initiate interdisciplinary projects.
• Identify patients who would benefit from skilled nursing/therapy and coordinate referrals to home care or community resources.
