Job summary

The University of Maryland Medical System is seeking a Primary Care Advanced Practice Provider to deliver comprehensive, patient-centered primary care in an outpatient setting, collaborating with physicians and the care team to promote preventive care and chronic disease management.

You will assess and manage patients across the lifespan, coordinate care, and participate in health promotion and continuity of care, while adhering to licensure, certification, and institutional policies.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current and unrestricted state licensure as a Nurse Practitioner or Physician Assistant.
• Current national certification through an approved certifying body (ANCC, AANP, or NCCPA).
• Current Basic Life Support (BLS) certification required.
• Prior experience in primary care, family medicine, internal medicine, ambulatory care, or community health preferred.
• Experience managing chronic medical conditions and preventive care services preferred.
• Experience working with diverse patient populations preferred.

Responsibilities

• Evaluate, diagnose, treat, and manage patients across the lifespan in the outpatient primary care setting.
• Emphasize preventive care, chronic disease management, health promotion, and continuity of care.
• Collaborate with physicians and the interdisciplinary healthcare team to deliver patient care.
• Coordinate care with nurses, care managers, behavioral health professionals, pharmacists, and community resources.
