Bookmark Medical is seeking an experienced Primary Care Advanced Practitioner (NP or PA) to join our established Primary Care team in White House, TN. You will provide comprehensive care to a diverse patient population, perform exams, diagnose conditions, and develop treatment plans.

Qualifications

• Graduate of an accredited NP or PA program and current certification/licensure.
• At least one year APP experience, preferably in primary care.
• Strong clinical skills and EMR proficiency.

Responsibilities

• Provide comprehensive care to a diverse patient population
• Conduct thorough physical exams, diagnose medical conditions, and develop personalized treatment plans
• Promote health education and disease prevention
• Order and interpret diagnostic tests and manage chronic conditions
• Collaborate with specialists and other healthcare providers to ensure coordinated care
• Maintain accurate up-to-date medical records and patient charts
• Stay current with medical advancements and best practices in Primary Care
