Bookmark Medical in Springfield, TN is seeking an experienced Primary Care Advanced Practitioner (NP/PA) to join our established Primary Care team. You will provide comprehensive care, conduct exams, diagnose conditions, and develop individualized treatment plans in a collaborative, patient-centered environment.

This full-time role offers a salary range of $115,000–$145,000 plus incentives, access to Navina, Ambience, and Athena Health to streamline documentation, and a broad benefits package

Qualifications

• Graduate of an accredited NP or PA program; current certification and licensure.
• At least one year of APP experience, preferably in primary care.
• Strong clinical skills, collaborative mindset, and proficiency with EMR systems.

Responsibilities

• Provide comprehensive care to a diverse patient population.
• Conduct thorough physical exams, diagnose medical conditions, and develop personalized treatment plans.
• Promote health education and disease prevention.
• Order and interpret diagnostic tests and manage chronic conditions.
• Collaborate with specialists and other healthcare providers to ensure coordinated care.
• Maintain accurate up-to-date medical records and patient charts.
• Stay current with medical advancements and best practices in Primary Care.
