Job summary

Responsibilities

• Evaluate, diagnose, and manage patients with acute illnesses, chronic diseases, and preventive care.
• Develop individualized treatment plans including medications, immunizations, and health screenings.
• Order and interpret labs and imaging to support clinical decisions.
• Perform office-based procedures within scope of practice.
• Conduct wellness visits and chronic care management with evidence-based guidelines.
• Coordinate referrals and collaborate with physicians, nurses, and other providers.
