Job summary

Northeast Healthcare Recruitment, Inc. is seeking a Primary Care Nurse Practitioner to join a collaborative, patient-centered team in Chula Vista, CA.

You’ll see 10–15 patients per day with longer visits and a focus on preventive care and whole-person wellness. Qualifications include NP Board Certification in primary or family/internal medicine, current ARNP license and DEA, and English/Spanish bilingual proficiency.

Qualifications

• NP Board Certification in primary care, family medicine or internal medicine.
• Current, valid and unrestricted ARNP license and DEA number.
• English/Spanish bilingual proficiency is essential.
• Minimum 3–5 years of primary care experience.
• Minimum 2+ years with case management emphasizing health prevention and population health management.
• Documentation of a negative TB test within the last 12 months prior to start date.
• Team-based care experience.
• Experience working in EMR systems.
• Experience with Google Workspace.

Responsibilities

• Provide primary care within a collaborative, team-based care model.
• Deliver longer visits focused on quality and patient relationships.
• Support prevention, wellness, and population health goals in daily practice.
