Greater Good Health is seeking a Nurse Practitioner to deliver patient-centered care to Medicare‑Medicaid dual eligible populations in a San Bernardino clinic. This full-time role emphasizes thorough assessments, education, and collaboration with a care team to improve outcomes.

The position requires NP certification (ANCC/AANP), 2+ years of clinical experience, DEA prescriptive authority, CPR/BLS certification, and bilingual English/Spanish proficiency.

Qualifikationen

• Nurse Practitioner certification from ANCC or AANP.
• 2+ years of NP clinical experience in Primary Care/Family/Internal/Gerontology/Palliative.
• Knowledge of chronic care management and medications.
• Current DEA license and prescriptive authority.
• Active CPR/BLS Certification.
• Bilingual English/Spanish.
• Experience with Medi-Medi/dual-eligible or underserved populations preferred.

Aufgaben

• Manage patients with chronic conditions including diabetes, hypertension, CKD.
• Performs Comprehensive Health Assessments and follow‑ups, reviews labs and vitals, and adjusts treatment plans.
• Provides patients with education on preventive measures, lifestyle and disease management.
• Support highly complex patients navigating the healthcare system with one‑on‑one care.
• Collaborate with healthcare team to coordinate high quality, cost‑effective care.
• Provide coordinated care between patient, family, physicians and other care team members.
• Administer various tests and screenings.
• Maintain strict confidentiality in line with HIPAA regulations.
