Job summary

HealthPoint in Des Moines, WA is seeking a Mental Health Therapist to deliver integrated mental and behavioral health care within a primary care team. You will assess and treat mental health and substance use concerns, maintain caseloads, document in EMR, and collaborate with medical colleagues to provide team-based care.

Requirements include a Master’s degree in Social Work or Counseling, licensure, and experience with underserved populations; vaccination and immunization records are required.

Qualifications

• Master’s degree in Social Work or Counseling required.
• 1–3 years of experience with underserved or high-risk populations.
• Integrated care training or experience in primary care mental health.
• Current licensure as LSW/LSW-Associate; LMHC/LMHC-Associate; or LMFT/LMFT-Associate.
• supervision for licensed status within 6 months.
• Team collaboration ability.
• Comfort with fast pace of primary care.
• Effective communication with primary care providers.
• Engage patients therapeutically.
• Experience with screening/assessments for mental/substance use.
• Knowledge of differential diagnosis for mental disorders.
• Knowledge of evidence-based psychosocial treatments.
• Experience with brief counseling techniques (MI, Behavioral Activation).
• Basic knowledge of psychopharmacology.
• Knowledge of DSM-5 and chemical dependency.

Responsibilities

• Assist primary care providers in recognizing and treating mental health and substance use in a primary care setting.
• Conduct initial patient visits and provide evidence-based interventions within a collaborative care model.
• Maintain a caseload of patients needing ongoing mental health interventions and outreach.
• Document services in EMR and MHITS and support grant work via tracking systems.
