Job summary

OhioHealth is seeking an LPN to join our Primary Care team in a day-shift outpatient setting. The role involves providing nursing care to patients, performing assessments, and collaborating with providers, nurses, and medical assistants to deliver high-quality care.

New grads are welcome. You will gain exposure to procedures such as EKGs and urinalysis, while contributing to a supportive, patient-focused environment within a busy medical office.

Qualifications

• Must hold a current Ohio license as an LPN.
• BLS certification is required.
• Strong patient-focused, team-based care mindset.

Responsibilities

• Perform initial and ongoing health assessments and document findings.
• Plan and coordinate nursing interventions and care plans.
• Evaluate patient responses to interventions and adjust care as needed.
• Delegate tasks to UAPs per state regulations and supervise as appropriate.
• Participate in process improvement and multidisciplinary team activities.
