Job summary

OhioHealth is seeking an LPN to provide nursing care to patients and families along the health-illness continuum in diverse settings, collaborating with the health care team.

The LPN is accountable for nursing practice as defined by the Ohio Board of Nursing and delegates tasks to UAPs per the Ohio Nurse Practice Act. This role serves as a resource and may assist with scheduling and inspections. No supervisory responsibilities are involved.

Qualifications

• High School or GED is required.
• BLS certification by American Heart Association.
• LPN license by the state boards of nursing (Ohio).

Responsibilities

• Initial, ongoing health status assessments and data collection from patients and families; communicate findings to the care team.
• Develop and coordinate care plans, initiate interventions, and promote cost-effective, coordinated care.
• Evaluate responses to nursing interventions and adjust plans as needed within a multidisciplinary team.
• Delegate tasks to unlicensed personnel per state criteria and supervise their work under established guidelines.
• Participate in process improvement activities to enhance clinical quality and patient safety.
