Job summary

OhioHealth in Mansfield is seeking an LPN to provide nursing care across the health-illness continuum within diverse care settings. Qualifications

• High School or GED (Required)
• CPR - Cardiopulmonary Resuscitation
• LPN - Licensed Practical Nurse - National Council of State Boards of Nursing
• Must hold a current license from the State of Ohio
• Ohio Nurse Practice Act knowledge

Responsibilities

• Perform initial, ongoing and functional health status assessments.
• Document planned nursing interventions and coordinate care plans.
• Delegate tasks to unlicensed assistive personnel (UAPs) and supervise as appropriate.
• Evaluate responses to interventions and revise the care plan accordingly.
• Participate in process improvement and quality initiatives.
