Job summary

Virtua Health is seeking an LPN to support primary care clinics in New Jersey. You will assist with patient intake, exams, vaccines, and essential care coordination, under clinician supervision.

Qualifications

• 6 months of experience in a physician practice preferred.
• Knowledge of minor surgical procedures and sterile technique.
• Graduate of approved LPN School of Nursing.
• Current NJ LPN license and CPR certification required.

Responsibilities

• Collaborates with Practice Leadership to conduct Daily Huddles and Weekly Care Team meetings.
• Prepares patients for exams by obtaining vital signs, documenting chief complaints, and preparing for provider evaluation.
• Prepares exam rooms and assists with examinations as required.
• Provides general medical treatments/orders as directed by physicians, including medications and dressings.
• Performs phlebotomy, vaccines, and diagnostic tests (e.g., EKG, pulmonary function) as ordered.
• Supports Population Health and care coordination through data review and patient follow-up.
• Leads patient education efforts and trains Medical Assistants in health coaching and education delivery.
• Coordinates transitions of care, including timely outreach after discharge or ED visits and EMR integration of records.
• Maintains clinical supplies and conducts daily regulatory testing to ensure equipment accuracy and compliance.
• Assists with Coumadin management and ongoing patient follow-up.
