OnePeak Medical in Oregon is seeking a Medical Provider to deliver comprehensive care across office visits and telehealth, emphasizing whole-person, functional medicine. You will build lasting patient relationships and collaborate with a high-functioning care team to optimize health outcomes.

The role offers a guaranteed salary with earning potential of $300k+ and relocation support, along with ongoing training and strong clinical support. A patient-centered, forward-thinking practice awaits.

Qualifications

• Active Oregon license as a Nurse Practitioner*, PA-C, MD, or DO (or eligibility to obtain).
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
• Proficient in using electronic health records
• Nurse Practitioner Licenses Accepted:
• FNP, Family Nurse Practitioner
• AGNP-PC, Adult Gerontological Nurse Practitioner (Primary Care)

Responsabilités

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment
