OnePeak Medical is seeking a dedicated Medical Provider to join our Southern Oregon clinics in Talent, Medford, Central Point, Grants Pass, and White City. You will deliver comprehensive medical services, build lasting patient relationships, and collaborate with a high-performing care team to achieve optimal health outcomes.

The role emphasizes whole-person care, preventive strategies, and integrative medicine, with a guaranteed salary and potential for growth.

Qualifications

• Active Oregon medical license in good standing for NP/PA/MD/DO.
• Board certification in Family Medicine or Internal Medicine.
• Strong communication and patient relationship skills.
• Proficient with electronic health records (EHR).

Responsabilités

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans including lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment

Compensation

Guaranteed salary with earning potential of $300k+, relocation offered

Responsibilities And Duties

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR in a timely and compliant manner

Required Qualifications

• Active Oregon license as a Nurse Practitioner*, PA-C, MD, or DO (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
• Proficient in using electronic health records
• Nurse Practitioner Licenses Accepted:
• FNP, Family Nurse Practitioner
• AGNP- PC, Adult Gerontological Nurse Practitioner (Primary Care)
