OnePeak Medical is seeking a passionate Medical Provider to join our growing team. This role offers the opportunity to deliver comprehensive medical services, build lasting patient relationships, and collaborate with a high-functioning care team to drive optimal health outcomes.

Formación

• Active Oregon license as NP/PA/MD/DO or eligibility to obtain.
• Board certification in Family Medicine or Internal Medicine.
• Strong interest in preventive, functional, and lifestyle medicine.
• Excellent communication and patient relationship skills.
• Proficient in using electronic health records.

Responsabilidades

• Provide direct patient care through office visits and telehealth.
• Perform comprehensive assessments and order labs/imaging as needed.
• Develop personalized treatment plans including lifestyle interventions.
• Educate patients on preventive health and functional medicine principles.
• Collaborate with health coaches, nutritionists, and other staff.
• Document visits in the EHR accurately and timely.
• Participate in meetings, quality improvement, and training sessions.
• Support clinic operations and foster a positive team environment.

Compensation

Guaranteed salary with earning potential of $300k+, relocation offered

Responsibilities And Duties

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments


• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions


• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions


• Educate and empower patients on preventive health and functional medicine principles


• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff


• Document visits in the EHR in a timely and compliant manner


• Participate in team meetings, quality improvement initiatives, and training sessions


• Support clinic operations by contributing to a positive, team-oriented environment

Required Qualifications

• Active Oregon license as a Nurse Practitioner*, PA-C, MD, or DO (or eligibility to obtain)


• Board certification in Family Medicine, Internal Medicine, or applicable specialty


• Strong interest in preventive, functional, and lifestyle medicine


• Excellent communication and patient relationship skills


• Comfortable working in a fast-paced, collaborative environment


• Proficient in using electronic health records


• Nurse Practitioner Licenses Accepted:


• FNP, Family Nurse Practitioner


• AGNP- PC, Adult Gerontological Nurse Practitioner (Primary Care)
