OnePeak Medical is seeking a licensed Nurse Practitioner to join our Primary Care Functional Medicine team in Grants Pass, Oregon. You will deliver comprehensive medical services, utilize a root-cause medicine approach, and collaborate with a high-functioning care team to improve patient outcomes.

Offer includes guaranteed salary with earnings potential of $300k+, relocation assistance, and a 4-day work week with no nights or weekends.

Qualifikationen

• Active Oregon license as NP/PA/MD/DO or eligibility.
• Board certification in Family/Internal Medicine or related specialty.
• Strong interest in preventive, functional, and lifestyle medicine.
• Excellent communication and patient relationship skills.
• Proficient in using electronic health records.

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments.
• Perform comprehensive assessments, order labs/imaging, and diagnose acute and chronic conditions.
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions.
• Educate and empower patients on preventive health and functional medicine principles.
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff.
• Document visits in the EHR in a timely and compliant manner.
• Participate in team meetings, quality improvement initiatives, and training sessions.
• Support clinic operations by contributing to a positive, team-oriented environment.

Compensation: Guaranteed salary with earning potential of $300k+, relocation offered

RESPONSIBILITIES AND DUTIES:

REQUIRED QUALIFICATIONS

• Active Oregon license as a Nurse Practitioner *, PA-C, MD, or DO (or eligibility to obtain).
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
