Job summary

OnePeak Medical Providers is seeking a Nurse Practitioner to join our Roseburg clinic in Oregon. You will provide comprehensive primary care with a focus on root-cause and functional medicine, collaborating with a high-functioning care team to ensure optimal patient outcomes.

The role offers comprehensive training, no night shifts, a 4-day work week with a flexible schedule, and strong clinical support including a medical assistant and scribe. Relocation assistance is available.

Qualifications

• Active Oregon NP license or eligibility
• Board certification in Family or Internal Medicine
• Strong interest in preventive and functional medicine
• Experience with electronic health records (EHR)

Responsibilities

• Provide direct patient care through office visits, telehealth, and same-day appointments
• Perform comprehensive assessments, order/interpret labs/imaging, diagnose conditions
• Develop personalized treatment plans including lifestyle interventions and prescriptions
• Educate patients on preventive health and functional medicine
• Collaborate with health coaches, nutritionists, mental health providers, and staff
• Document visits in the EHR in a timely, compliant manner
• Participate in team meetings and quality improvement initiatives
• Support clinic operations and maintain a positive team environment

Compensation: Guaranteed salary with earning potential of $300k+, relocation offered

RESPONSIBILITIES AND DUTIES:

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment

REQUIRED QUALIFICATIONS

• Active Oregon license as a Nurse Practitioner *, PA-C, MD, or DO ( or eligibility to obtain )
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
