Job summary

OnePeak Medical is seeking a skilled Nurse Practitioner to join our Lake Oswego clinic, delivering comprehensive primary care with a functional medicine focus. You’ll build lasting patient relationships, perform office and telehealth visits, and collaborate with a high-functioning care team to optimize wellbeing.

Qualifications

• Active Oregon NP/PA/MD/DO license or eligibility to obtain
• Board certification in Family Medicine or Internal/Internal Medicine
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Proficient in using electronic health records

Responsibilities

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, diagnose acute and chronic conditions
• Develop personalized treatment plans including lifestyle interventions, supplements, hormones, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment

RESPONSIBILITIES AND DUTIES:

REQUIRED QUALIFICATIONS

• Active Oregon license as a Nurse Practitioner *, PA-C, MD, or DO ( or eligibility to obtain )
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
