Qualifications

• Active license as NP in Oregon or eligibility to obtain
• Board certification in Family Medicine or Internal Medicine
• Minimum 2 years clinical experience; functional/integrative medicine preferred

Responsibilities

• Provide direct patient care via office visits and telehealth
• Order/interpret labs and imaging; diagnose conditions
• Develop personalized treatment plans (lifestyle, hormones, prescriptions)
• Educate patients on preventive health and functional medicine
• Collaborate with health coaches, nutritionists, mental health staff
• Document visits in NextGen EHR timely and compliant
• Participate in team meetings and quality improvement
• Support clinic operations and foster a team-oriented environment

RESPONSIBILITIES AND DUTIES:

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR (NextGen) in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment

REQUIRED QUALIFICATIONS

• Active Oregon license as a Nurse Practitioner *, PA-C, MD, or DO (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Minimum 2 years of clinical experience preferred (functional/integrative medicine experience is a plus)
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
• Proficient in using electronic health records (NextGen experience is a plus)
